Sarah Godfrey's Portfolio

Gerontology Clinical

  1.  Benedictine Living Center- Administered medications, assisted with ADL's, and  vitals, conducted full assessment exams, assisted with new stroke patient education and ADL's administered insulins, took blood sugars - 40 hours
  2.  Annunciation Monastery - Assisted with medication administration, including subcutaneous injections, assisted in some ADL's, managed medications and assisted with scheduling- 3.5 hours

Medical Surgical Clinical

- Sanford Medical Center 

  1.  ICU step-down unit - complete tracheostomy care for new tracheostomy, including in-line suction, administered tube feeding through NG tubes,  managed chest tubes and drain including administering TPa via chest tube, managed and changed IV medications and pumps- 36 hours
  2. Pre/Post operation -   6 hours
  3. Respiratory Therapy - Shadowed a respiratory therapist and administered different inhaled medications through a nebulizer in a hospital setting - 3 hours


-CHI St. Alexius Health Bismarck Medical Center 

  1. Medical Surgical - Managed post-surgical patient with hip fracture, managed JP drain, surgical sites, administered medication through PICC, and managed the patient with Guillain-Barre Syndrome with a tracheostomy. 36 hours
  2. Operating Room - Observed a bilateral breast tissue expander removal excision muscle-sparing transverse rectus abdominis myocutaneous surgery. 8 hours


Women's Health

- Sanford Medical Center 

  1. Labor and Delivery - Took care of labor mothers through induction and birth - Managed IV pitocin, fluids, epidural pump, IV placements, and fluids.  12 hours
  2. Mom and Baby -Took care of mother and infant with grade 4 tongue tie. Conducted hearing screen and newborn exam and assisted with tongue tie operation. Educated about nursing and aftercare of surgery, and monitored mother's uterine and psychological needs. - 12 hours

- Essentia Health Mid Dakota Women's Center-Bismarck 

  1. Women's Health - Shadowed a nurse and assisted with rooming patients, vitals, and discharge instructions for pregnant women, patients needing annual screenings, or problem-focused gynecological needs.  4 hours

Mental Health

1. Youth Correctional Center and Rehabilitation - Administered medications, examined injuries, educated on wound healing, and assisted with orthopedic exam and wound- 8 hours

2. North Dakota State Hospital - Assisted with therapeutic communication with acute psychiatric, and learned techniques to manage escalating issues within wards with new patients suffering from acute psychosis -  patient-  10 hours

3. Mental Health First Aide - Attend live webinar for training of Mental Health first aid and attained certification - 8 hours

Community Health

1. UND Center for Family Medicine Clinic - Assisted with patient admits vitals, and education - 8 hours

2. St. Alexius CHI Flu Vaccine Clinic - Administered flu vaccines to CHI staff - 4 hours

3. North Dakota  Corrections and Rehabilitation (State Penitentiary) - Shadow nursing staff, performed injections, assisted with tube feeding, and medication administration.  8 hours

4. North Dakota Corrections and Rehabilitation (State Penitentiary) Flu Vaccine Clinic - Administered flu vaccines to inmates. - 4 hours

5. CHI Home Health - Wound care pressure sores on heals, sacrum and coccyx, central veinous line dressing change on pediatric, antecubital blood draws - 8 hours

Critical Care

1. Sanford Emergency Department - Assisted with cardiac patients, respiratory distress, pediatric, including fractures and pneumonia, and rheumatic fever, severe frostbite. Administered medication, started IVs, dressed wounds, placed slings, and dealt with reporting abuse. 24 hours

2. St. Alexius CHI ICU- Managed cardiac intubated patients, rhabdomyolysis patients, post-cardiac mitral valve replacement patients, and acute respiratory failure. Titrated medications, placed OG tubes, transesophageal temperature probes, foley catheters with temperature probes, and maintained arterial monitor lines, managed airways and suctions as needed.  36 hours

Pediatrics

1-Sanford Pediatrics - Assisted with cares for a child with neonatal Marfan syndrome who was experiences acute respiratory failure due to viral RSV. Managed care for a child who was diagnosed with strep A with abscesses located on the lateral side of the left neck and posterior about C3. Cares included, intravenous and oral medications administration, frequent assessments, family assurance and assistance with follow-up appointments, growth and development activities. - 12 hours

2- Anne Carlson - I attended clinical at Anne Carlson and was able to care for 5 different patients who were severely disabled. We straight catheterized multiple patients, worked with G,J,C, tubes for feeding and bowel management. Placed a flexi-seal, managed trachs, including replacement of a trach, and  medication administrations. - 8 hours

3- Bismarck Public School Nurse - This was a fun experience. I was able to shadow a nurse between two elementary schools and assisted with vision screenings, medication administration, diabetic management, and working with various child situations.  - 8 hours

Simulations

  1. End-of-Life - Taking care of an in-patient who was dying and managing care and family - 4 hours
  2. Adaptive (gerontology) - Navigating different disabilities through the day and performing normal ADLs. 4 hours
  3. Long-term care admissions - Small Group Session regarding a patient's long term care admission  - 4 hours
  4. Diabetic Management - Taking care of an in-patient diabetic who is suffering from wounds and pre-surgery- 4 hours
  5. Gastrointestinal Bleed - Taking care of an in-patient who presented with vague symptoms after surgery and replacing blood. -4 hours
  6. Obstetrics - Labor and Delivery - Taking care of laboring patient and resuscitating a newborn 4 hours 
  7. Domestic Violence - Navigating a scenario of an abuse victim through community services - 4 hours
  8. Community SIMS - Home Health (PICC) Caring for a patient at home who has a PICC line, wound after a below-the-knee amputation because of osteomyelitis, and had become non-compliant with IV medications and now has become septic- 4 hours
  9. Poverty SIMS - Navigated  a scenario of a single mom with kids that's been abandoned through community services and surviving a month- 8 hours 
  10. Disease Outbreak SIMS - Community Health - Navigated an ICS command disease outbreak of Meningitis on Campus and worked through the different operations and positions, including public relations media, PODS with patients, contact trace calling, and financial audit - 4 hours
  11. Skill Blitz - This was a fun SIMS, where we practiced intravenous placements, pump setups, foley placement, nasogastric tube placement in children, tracheostomy cares and suctioning, and in different seniors - 4 hours
  12. Critical Care - Emergency Cardiac Resuscitation - We had to work through a patient destabilizing and going into supraventricular tachycardia, use the AER device and sync, perform CPR, administer drugs, breathing, record, and defibrillate the patient as a code team with assignments, and navigate communication with providers and family. 
  13. Pediatric Appendicitis SIMS- Worked as a team to care for a 8 year old child who presented for appendicitis that had ruptured. Managed medications, MAR, and provide education and comfort to mother of child.
